A few things are worth noting, some of these may or may not apply to your particular case.

1. Sometimes the pelvic opening is just the wrong shape, this is one reason why some women have to have Ceasarean deliveries in childbirth. The bone won't move and if there to little room for the hand, it won't go in.
2. Positioning of the hand/fingers is really important but no one set of positioning is the magic answer for everyone, each woman is shaped a little different.
3. The angle can be a factor, if you have tried this laying down and got stuck try doing it in a doggy style position, if you have tried doggy style try laying down, you can also try laying on your side. Your particular vagina is at an angle that is unique to you so you need to experiment to find the optimum angle
4. Sometimes very gently rotating the hand back and forth at the point where you are "so close" but before the point that it "starts to hurt to much" will help.
5. Being very aroused before hand, often helps
6. Be sure not to try to close the fist to early, the tumb is best tucked up in a groove created by the other four fingers - this makes for a tapered, flexible shape to the hand, only begin to curl the fingers into a fist when insertion has gone far enough to reach the large secondary chamber immediately ahead of the cervix.
7. Relaxing breathing techniques on your part will help. Your partner can coach your breathing. You want to breath deeply filling your tummy not your chest. Slowly inhale for perhaps a long three count, hold the breath without straining for perhaps three more counts and then exhale slowly for perhaps a count of six (it usually takes longer to release the breath than to draw it in.) Experiment with the exact count until you find a rythym that is relaxing for you. Practice the breathing and get it down pat before combining it with the fisting itself.
8. Be patient, often just holding the hand motionless at the "almost in but not yet hurting too much" point will give the muscles time to relax - this can take a couple of minutes (which will seem like a very long time if you are just holding still).
9. Consider finding a trusted friend with a smaller hand than your partner - this may help you transition from the smaller to the larger, once you have success with a smaller hand.

it just takes some time and practise...try one finger at a time, finding a comfort level...when that is reached, add another finger...keep this up until the maximum. don't be impatient...Y/you'll get there.

heck, you can even make one yourself
if you are an arts and crafts person.
and besides, the incentive to make one
which fits better will keep your
attention focused.
